About this review
Today the Mid and West Wales Regional Safeguarding Board publishes the report of the Extended Adult Practice Review, CWMPAS 5 2018, in respect of a 22-year-old young person from Powys who sadly died in 2016.
The Review has been undertaken in accordance with statutory legislation set out in the Social Services and Wellbeing (Wales) Act 2014 and accompanying guidance Working Together to Safeguard People – Volume 3 – Adult Practice Reviews (Welsh Government, 2016).
The Mid and West Wales Regional Safeguarding Board will oversee the delivery of an action plan and are committed to ensuring that lessons continue to be learnt and services improved.
