CYSUR 1 2021 Concise Child Practice Review
Today the Mid and West Wales Regional Safeguarding Board publishes the report of the Concise Child Practice Review, CYSUR 1 2021 in respect of a young child who resided in Pembrokeshire, and who tragically died on 21st July 2020.
The Review has been undertaken in accordance with statutory legislation set out in the Social Services and Wellbeing (Wales) Act 2014 and accompanying guidance Working Together to Safeguard People – Volume 2 – Child Practice Reviews (Welsh Government, 2016).

The dissemination event will share learning that has emerged from the Child Practice Review CYSUR 1/2021 in respect of child A, a young child who resided in Pembrokeshire, and who tragically died in July 2020 following an unprovoked and violent attack.
The review explored themes relating to information sharing, assessments, supervision, and concerns which fall below the threshold of significant harm. The learning identified in this review is relevant for all statutory agencies who work in child safeguarding.
