Today the Mid and West Wales Regional Safeguarding Board publishes the report of the Concise Child Practice Review, CYSUR 1 2024, in respect of a child in Pembrokeshire who tragically died in January 2024.
The Review has been undertaken in accordance with statutory legislation set out in the Social Services and Wellbeing (Wales) Act 2014 and accompanying guidance Working Together to Safeguard People – Volume 2 – Child Practice Reviews (Welsh Government, 2016).

The dissemination event will share learning that has emerged from the Child Practice Review CYSUR 1/2024 concerning Child A, who was tragically killed by his mother whilst she was experiencing a psychotic episode. The review explored themes relating to elective home education, the voice of the child, and contextual safeguarding. The learning identified in this review is relevant for all statutory agencies who work with and support children and families.
